The amplifier carries a 60 Day Warranty, this date is effective 60 days from the actual ship date. If your item is no longer under this warranty, then it will not be repaired or replaced. If you choose to mail it to us anyway, then it will only be returned to you at your expense.

If the defect is determined to be a product of misuse, neglect, mistake, or modification, the repair may incur the cost of the replacement part/s and labor.

The Amplifiers require special hookup instructions. There are two power wires and one ground wire, if these are not hooked up correctly then the amplifier will not function. Please check the connections before you complete this form.

This form must be filled out completely before a refund will be issued, any fields that are left blank may cause a delay in processing your repair.
